| Completed and submitted on-line the registration form by May 10, 2016. Note: there is 30 min allocated to complete the form before the session timed out | |
| Identified ALL special requests on the registration form (e.g. late arrival or early departure, PD days, holidays, etc.) | |
| Registration e-mail received – sent automatically a few moments after the submission of the registration form on-line. If no confirmation e-mail was received it means that the registration has not been sent. This may be due to the session having timed out if it took more than 30 minutes to complete the registration form | |
| Sent $100 cheque deposit to: P.A.R.T.Y. Program – The Ottawa Hospital, Civic Campus, Trauma Services CPC-1 R164, 1053 Carling Ave. Ottawa, ON, K1Y 4E9 |
