What is a “Family Health Team”?
The Ottawa Hospital Academic Family Health Team is a group of health-care professionals working together for the benefit of their patients. We provide primary care for teenagers, children, the elderly, pregnant mothers, babies, and the general population.
Our team is composed of a variety of professionals, including:
- Nurse Practitioners
- Physician Assistants
- Social Workers
- Administrative Staff
- And more!
Our clinic provides a complete range of health-care services, including nutrition advice, drug information, counselling sessions, and mental health services, all designed to assist our patients in attaining and maintaining optimal health.
We Are a Teaching Hospital!
Our team is called the Ottawa Hospital Academic Family Health Team. “Academic” means that we are a teaching hospital. This means we are partnered with the University of Ottawa to train medical students, residents, and other health-care professionals to the highest standard.
Because we are a teaching clinic, sometimes you may be seen by a resident. Residents are doctors who have finished their medical degree and are completing hands on post-graduate training. Residents who see patients are directly supervised by the clinic’s family doctors.
For more information, please visit Our Teaching Environment.
Are You Currently a Patient?
Here is some important information you need to know:
- Our team uses Electronic Health Records so your health information is easy to access when you need to be seen by other health-care providers within our clinic.
- Enrollment: Our clinic is registered as an Ontario Family Health Organization (FHO), so we ask that our Ontario patients “roster” with us. This involves signing a form, and just means that you are identifying the doctors at our clinic as being your main health-care providers. This does not put your under any obligation, and is free of charge.
Are you Interested in Becoming a Patient?
The Family Health Team is not a referral service or a walk-in clinic. Only patients who have their Family Doctor at the clinics are seen.
If you do not currently have a family doctor in Ottawa, Health Care Connect may be able to help. Health Care Connect helps people from Ontario who do not have a family health-care provider (family doctor or nurse practitioner) to find one.
Please visit their website: http://www.ontario.ca/health-and-wellness/find-family-doctor-or-nurse-practitioner or give them a call at: 1 (800) 445-1822.
