Shared Mental Health Care Team–TOH and Bruyere Academic Family Health Teams (FHT’s)
- Clinical practice: clinical expertise in the care of clients living with mental illness, and their families
- Consultation: advanced knowledge of treatment options for clients with mental illness
- Education: knowledge sharing of mental health issues, current treatments, symptom managements and factors impacting quality of life for patients and families, frequent provision of in-services to the RN group, clerks and allied health groups of each of the four FHT’s serviced by the SMH team. Also providing in-services to the Faculty of Medicine clerks within the Wellness Program affiliated with the University of Ottawa
- Leadership: manager of the Shared Mental Health Care (SMH) Team with 5 direct reports, liaison between members of the SMH team and members of the FHT’s, and interprofessional collaboration with all members of the healthcare team
- Research: co-author on abstracts and posters, research exploring best practice in the delivery of support to family physicians with results accepted for presentation at the National Collaborative Care Conference. Spearheading several quality initiatives including: the optimization of the referral process; exploration of factors contributing to high rates of referrals with results used to guide capacity building activities; optimizing intake templates to maximize their usefulness and efficiency; exploration of interim supports that can be offered to patients as they face increasingly long wait times for direct services. Research interests include stigma reduction, use of on-line resources and guided self-care in supporting clients managing their chronic conditions
