Quality Improvement Plan 2016

April 1, 2016 In compliance with the Ontario’s Excellent Care for All Act which came into law in June 2010, The Ottawa Hospital (TOH) is pleased to share its Quality Improvement Plan. Submitted to the Ontario Health Quality Council, TOH’s publicly reported plan outlines improved targets in the areas of safety, effectiveness, efficiency, access and patient-centeredness.

To achieve its goal of becoming a top North American hospital in terms of quality and safety of patient care, The Ottawa Hospital will focus on several key areas. TOH will work to improve patient satisfaction and reduce wait times in the Emergency Department while reducing the time patients spend in acute care when they no longer need a hospital bed.

Improved medication reconciliation upon admission and discharge and reduced rates of C. Difficile and other infections remain a priority for patient safety. TOH will continue to improve hospital effectiveness by reducing unnecessary readmissions and preventable deaths, especially for individuals with Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD).
