We are the Shared Care Mental Health Program!

We are a multi-disciplinary team of mental health professionals, including psychiatrists, two social workers, an Advanced Practice nurse, a Registered Nurse, and an Administrative Coordinator. We work closely with your health-care provider to ensure that you receive the mental health support and treatment you need. At this time, we only see Ontario patients who are 16 years and older who have been referred to us by a family doctor or resident.

Your plan may involve one or more of the following:

  • Individual therapy or counselling
  • Group therapy
  • Diagnostic clarification
  • Treatment recommendations
  • Education and advice about psychiatric medications
  • Information and help accessing community resources

We are able to see patients for up to 8 sessions, after which we will work closely with you to find alternative support if necessary. The Shared Mental Health program does not provide long-term follow-up services, nor are we able to provide you with a psychiatrist who will see you regularly over an extended time period.

The Family Health Team also offers Social Work Services!

At the Ottawa Hospital Academic Family Health Team, we also offer a variety of social work services to our patients. Our social workers provide counselling for individuals, couples, and families, and will refer to community resources / agencies when appropriate.

Our social workers can offer counselling, support, and information on:

  • Parenting concerns
  • Relationships
  • Coping with Grief or Illness
  • Managing Anger or Stress
  • Practical problem-solving (e.g: housing, finances, elder care)
  • Anxiety disorders
  • Depression and self-esteem

Please note that you do not need a referral to receive social work services. If you would like to book an appointment, please contact the front desk.
