Referral Information

A referral by a family physician or physician specialist is required. Referrals can be made by faxing or mailing a referral letter requesting a Physiatry assessment for the Chronic Pain Management Program to:

Please note that we do not treat patients:

• Covered under RAMQ (Quebec)
• Under the age of 18
• Without a valid health card

Also, we do not treat patients whose sole reason for referral and primary pain symptom is headache, GI/pelvic, gynaecological, or urinary pain, or for addictions management. Although these exclusion criteria do apply, all fully completed referral forms will be reviewed for suitability.
