The Referral Process

Inpatient Rehabilitation Referral Process

A referral is required to be assessed and to enter our treatment programs.

For patients currently admitted to a hospital, referrals should be sent through the Ocean e-referral website for the following inpatient programs:

  • Acquired Brain Injury (ABI) Service
  • Locomotor Care /Complex Orthopedics
  • Neuromuscular Care (including ALS, MS, Cerebral Palsy and spinal cord injuries)

For patients currently admitted to a hospital, referrals should be sent via fax for the following inpatient programs:

  • Amputee Rehabilitation: Dr. Nancy Dudek (Fax: 613-733-8336)
  • Respiratory Rehabilitation: Dr. Jackie Sandoz (Fax: 613-736-9054)

Patients being referred directly from the community must first be assessed by a The Ottawa Hospital Rehabilitation Centre Physiatrist in clinic.  Please fax a referral to 613-733-8336.

Each referral is reviewed by the rehabilitation team to determine if the program can help your specific needs so you can achieve your personal rehabilitation goals. The outcome of this review will be communicated directly to the physician or nurse who submitted the referral.

If you are on the waiting list you will be contacted as soon as a bed is available and an admission date will be set. Specific instructions for preparation for your inpatient stay will be given at this time.
