Refer a Patient to The Ottawa Hospital Breast Health Centre

You may refer a patient to The Ottawa Hospital Breast Health Centre for breast abnormalities such as:

• Suspicious lumps
• Nipple discharge
• Peau d’orange

Prior to referral:

• You must request appropriate imaging.  Fax a breast imaging requisition to 613-761-4405 or enter into CPOE if you have access.
• Patients greater than or equal to 35 years must have completed mammography and ultrasound
• Patients less than 35 years must have a completed ultrasound

Please provide if available:

• Chest X-ray
• Echocardiogram (measure ejection fraction)
• MUGA scan
• Imaging or diagnostic reports
