Overview

The Geriatrics program provides clinical services to patients over 65 years of age who are experiencing a change in function, memory, mood or have complex medical issues and who would benefit from a comprehensive Geriatric assessment. Our program offers in-patient, out-patient and community services.

Our Services

Patients are referred to Geriatrics for assessment of a variety of concerns such as:

  • Recent changes in physical, mental or functional abilities; falls, impaired mobility, dementia, delirium, change in mood, decreased the ability for self-care
  • Major changes in support needs, caregiver stress, institutional placement being considered.
  • Safety concerns – physical, psychological, social, environmental
  • Sudden increase in the use of health care services over the last 6 months.

In-Patient: Patients who are already in hospital at The Ottawa Hospital may be transferred to our Inpatient Geriatric Medicine Unit at the Civic Campus for assessment and treatment.  To be transferred to the unit patients must be referred by an in-patient physician and be seen in consultation by one of our Geriatric Nurse Specialists or Geriatric Physicians.

Out-Patient:  Patients are usually referred by their Physician or family.  Typically, patients have already been seen in their own home by the Geriatric Outreach Assessor before an appointment is made for the Geriatric Day Hospital.  At times, patients will be referred through the Emergency Department (Geriatric Emergency Management Program).

In partnership with the Regional Geriatric Program of Eastern Ontario, we offer a wide range of services to help patients and families make decisions to maximize their function and make decisions about their treatment and care.

The Regional Geriatric Program of Eastern Ontario aims to be a leader in the provision of:

  • Comprehensive, interdisciplinary geriatric assessment and treatment services in partnership with our patients and the community
  • Education in geriatrics/gerontology to practitioners, students and community members;
  • Research into appropriate treatment and services delivery to seniors; and
  • Advocacy and planning in the development of community services which promote seniors’ health.

Making an Appointment

In-patient: Referred by their hospital admitting doctor.

Out-patient:  Referrals to the Geriatric Assessment Outreach Team may be made by the physician, family or caregiver telephone, mail or fax Monday to Friday from 08:00 – 16:00.  The teams cover the Ottawa region. However, the family physician must be contacted before a referred will be accepted by the Outreach team.
