Acknowledgment of Funding Support

• The Ottawa Hospital receives funding from Champlain Local Health Integration Network. The opinions expressed in this publication do not necessarily represent the views of the Champlain Local Health Integration Network.
• Upon request of the Champlain LHIN, the Hospital will include a statement in a form acceptable to the Champlain LHIN, acknowledging the support of the Province.

Community Engagement and Integration.

• The Hospital will report annually on its community engagement and integration activities and at such other times as the Champlain LHIN may request from time to time, using any templates provided by the LHIN.
