{"id":68460,"date":"2021-09-24T13:00:26","date_gmt":"2021-09-24T17:00:26","guid":{"rendered":"https:\/\/www.ottawahospital.on.ca\/?p=68460"},"modified":"2021-10-21T11:12:59","modified_gmt":"2021-10-21T15:12:59","slug":"dental-clinic-provides-much-more-than-dental-care-to-patients-with-autism-cancer-and-other-complex-health-care-needs","status":"publish","type":"post","link":"https:\/\/www.ottawahospital.on.ca\/en\/healthy-tomorrows\/dental-clinic-provides-much-more-than-dental-care-to-patients-with-autism-cancer-and-other-complex-health-care-needs\/","title":{"rendered":"Dental clinic provides much more than dental care to patients with autism, cancer and other complex health-care needs"},"content":{"rendered":"\n<p>Jean-Marc Lang hasn\u2019t always liked going to the dentist\u2019s office.&nbsp; The 27-year-old has autism, epilepsy, a severe developmental delay and ritualistic behaviours that mimic obsessive compulsive disorder (OCD).&nbsp; For him, sitting in a dentist\u2019s chair for a cleaning seemed next to impossible.<\/p>\n\n\n\n<div class=\"wp-block-image is-style-default\"><figure class=\"alignright size-full is-resized\"><img decoding=\"async\" src=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Jean-Marc_Lang.jpg\" alt=\"Jean-Marc Lang watching a baseball game\" class=\"wp-image-68463\" width=\"187\" height=\"268\" srcset=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Jean-Marc_Lang.jpg 445w, https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Jean-Marc_Lang-209x300.jpg 209w\" sizes=\"(max-width: 187px) 100vw, 187px\" \/><\/figure><\/div>\n\n\n\n<p>\u201cJean-Marc has destructive rituals,\u201d explained his mother, H\u00e9l\u00e8ne. \u201cSo when things are not perfect, or not the same, he tends to want to damage them or get rid of them completely. He also has debilitating rituals where he will pick at his skin until it bleeds, or remove a nail.\u201d<\/p>\n\n\n\n<p>As Jean-Marc\u2019s behaviour intensified over the past decade, H\u00e9l\u00e8ne has become increasingly concerned with keeping Jean-Marc\u2019s teeth and gums healthy.&nbsp;<\/p>\n\n\n\n<p>\u201cHe has poor communication skills and is not able to say clearly if he\u2019s in pain or not, so that has always been a huge concern for us,\u201d she said. \u201cHe is sensitive to touch and noise, so any dental procedure\u2014an &nbsp;examination, cleaning or tooth repair\u2014has to be done very carefully to make the experience as comfortable as possible.\u201d<\/p>\n\n\n\n<p>When Jean-Marc was young, he first went to theCHEO dental clinic. Over time, thanks to skilled and caring staff, Jean-Marc began to feel more comfortable and eventually developed complete trust in the staff. Once he became an adult, he was referred to another excellent clinic in the community before being referred to the dental clinic at The Ottawa Hospital.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">A dental clinic like no other<\/h3>\n\n\n\n<p><a href=\"https:\/\/www.ottawahospital.on.ca\/en\/clinical-services\/deptpgrmcs\/clinics-and-services\/dental-clinic\/\">The Ottawa Hospital Dental Clinic<\/a> is unique in the region and provides specialized care to adult patients in hospital, as well as those who require care and resources that community dental clinics aren\u2019t able to provide.<\/p>\n\n\n\n<p>A staff of highly-trained oral surgeons, dentists, hygienists and support staff work closely together at the clinic to provide care to up to 40 patients per day who meet the <a href=\"https:\/\/www.ottawahospital.on.ca\/en\/clinical-services\/deptpgrmcs\/clinics-and-services\/dental-clinic\/\">eligibility criteria<\/a> and are referred by their doctor.&nbsp;<\/p>\n\n\n\n<p>\u201cConsider someone who has an uncontrolled seizure disorder, someone who is recovering from a heart transplant, or someone who has had a portion of their jaw removed as part of their cancer treatment,\u201d said Siobhan Little, Supervisor of The Ottawa Hospital Dental Clinic.&nbsp; \u201cThese patients require specialized care and resources that we can provide at our clinic. We can even go up to their room in the ICU if they have a concern.\u201d<\/p>\n\n\n\n<div class=\"wp-block-image is-style-default\"><figure class=\"alignleft size-full is-resized\"><img decoding=\"async\" src=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Dr._Howard_Weld_The_Ottawa_Hospital_Dental_Clinic-rotated.jpg\" alt=\"Dr. Howard Weld\" class=\"wp-image-68465\" width=\"217\" height=\"289\" srcset=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Dr._Howard_Weld_The_Ottawa_Hospital_Dental_Clinic-rotated.jpg 480w, https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Dr._Howard_Weld_The_Ottawa_Hospital_Dental_Clinic-225x300.jpg 225w\" sizes=\"(max-width: 217px) 100vw, 217px\" \/><figcaption>Dr. Howard Weld and the staff at The Ottawa Hospital Dental Clinic provide important dental care to people who have been referred by their doctor and need extra support.<\/figcaption><\/figure><\/div>\n\n\n\n<p>Many of the patients the clinic sees are either preparing for or recovering from radiation treatment for cancer.<\/p>\n\n\n\n<p>\u201cWe see patients who have head or neck cancer before and after their radiation,\u201d explained Dr. Weld, who has been an associate dentist at The Ottawa Hospital Dental Clinic since 2017.&nbsp; \u201cRadiation kills cancer, but it tends to also damage saliva glands, so mouths are very dry and teeth are prone to decay. We have to ensure that all the teeth that are left in a person\u2019s mouth are healthy and well cared for.\u201d<\/p>\n\n\n\n<p>The clinic also receives referrals for patients undergoing heart surgery who need to clear up an infection that could compromise the results of the surgery.<\/p>\n\n\n\n<p>&nbsp;\u201cThe reality is that we only have one bloodstream,\u201d he explained. \u201cIf there is infection in the mouth, there is infection in your body. Antibiotic drugs help, but until the source of infection is eliminated, it can return.\u201d<\/p>\n\n\n\n<p>Special resources also exist for patients with behaviour or anxiety issues.&nbsp; As part of their Dental Assist Program, hygienists are accompanied by an assistant, an extra layer of support for the patient.&nbsp; In some cases, patients can be gently wrapped in a \u201cpapoose\u201d during a procedure if it makes them feel more at ease. Funds will be used to purchase vital equipment for our dental clinic.<\/em><\/figcaption><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">A customized approach for every patient<\/h3>\n\n\n\n<p>With a strong sense of purpose and community support, the team at The Ottawa Hospital\u2019s Dental Clinic focuses on accommodating the needs of each patient.<\/p>\n\n\n\n<div class=\"wp-block-image is-style-default\"><figure class=\"alignright size-full is-resized\"><img decoding=\"async\" src=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Autistic_Patient_gets_dental_care-rotated.jpg\" alt=\"Jean-Marc wearing sunglasses and sitting in a dentist\u2019s chair at The Ottawa Hospital Dental Clinic\" class=\"wp-image-68471\" width=\"213\" height=\"284\" srcset=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Autistic_Patient_gets_dental_care-rotated.jpg 480w, https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/Autistic_Patient_gets_dental_care-225x300.jpg 225w\" sizes=\"(max-width: 213px) 100vw, 213px\" \/><figcaption>The staff at the dental clinic consult with patients like Jean-Marc and their families to learn how best to support each patient and make their experience as positive as possible<\/figcaption><\/figure><\/div>\n\n\n\n<p>\u201cWith every patient, we start with a consultation, and the treatment plan is made from there,\u201d explained Siobhan.&nbsp; \u201cIf they are deemed Hygiene Assist patients, we make sure that the extra person is there for support.&nbsp; If sedation is needed, we make sure all the proper consents are given and the paperwork is done.\u201d<\/p>\n\n\n\n<p>When Jean-Marc needed to come to the dental clinic to repair a chipped tooth, the clinic team met with his parents to discuss what it was like for him to go to the dentist in the past, and how he typically responds to certain situations, so that they could better anticipate what might happen during the appointment.<\/p>\n\n\n\n<p>With that knowledge, the team was ready to do what they could to make him feel as comfortable as possible.<\/p>\n\n\n\n<p>\u201cThey talked to him and tailored their approach as much as possible to meet his needs, making sure to build rapport and trust,\u201d said H\u00e9l\u00e8ne. \u201cThey helped him understand what was being done and what was expected of him during each step of the procedure.\u201d<\/p>\n\n\n\n<p>They also helped to make sure that Jean-Marc was comfortable.<\/p>\n\n\n\n<p>\u201cWhen I asked if there was something we could put on his legs to keep him warm, they came back with a warm blanket,\u201d said his mom. \u201dTheir level of kindness and compassion was so heart-warming.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Leveraging virtual dental care during the COVID-19 pandemic<\/h3>\n\n\n\n<p>When hospitals across Ontario were directed by the provincial government to pause non-urgent programs and services, the hospital\u2019s dental clinic scaled back to provide in-person dental care for emergency appointments only. But in an effort to provide as many patients as possible with timely care, the clinic also leveraged virtual care technology in some cases, which they plan to continue into the foreseeable future.<\/p>\n\n\n\n<p>\u201cVirtual care has been good for our clinic and our patients,\u201d said Siobhan.&nbsp; \u201cFor example, if we have a patient with a developmental delay, and we know that person will eventually need to go into the operating room, we don\u2019t need to have the patient and their two workers come to the clinic.&nbsp; Instead we can call their caregiver, potentially prescribe antibiotics and send off the paperwork to the operating room. It\u2019s a smoother process for the patient.\u201d<\/p>\n\n\n\n<p>As restrictions are lifted, the clinic is gradually increasing the number of services once again.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Grateful families, dedicated staff<\/h3>\n\n\n\n<p>Anyone who has been to a dental clinic knows that the staff can make the difference between a positive and a negative experience.&nbsp; In addition to being at the top of their game professionally, H\u00e9l\u00e8ne is pleased to say that the staff at the hospital clinic show incredible compassion.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-style-default\"><img decoding=\"async\" width=\"740\" height=\"330\" src=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/The_Ottawa_Hospital_Dental_Clinic.jpg\" alt=\"Two members of the dental clinic standing in a dental exam room at The Ottawa Hospital.\" class=\"wp-image-68461\" srcset=\"https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/The_Ottawa_Hospital_Dental_Clinic.jpg 740w, https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/The_Ottawa_Hospital_Dental_Clinic-600x268.jpg 600w, https:\/\/www.ottawahospital.on.ca\/wp-content\/uploads\/2021\/09\/The_Ottawa_Hospital_Dental_Clinic-300x134.jpg 300w\" sizes=\"(max-width: 740px) 100vw, 740px\" \/><\/figure>\n\n\n\n<p class=\"has-text-align-center\"><em>The team at the clinic work together to provide each patient with world-class, compassionate care.<\/em><\/p>\n\n\n\n<p>\u201cThey were so kind and patient, so open-minded and flexible as well as efficient and professional,\u201d she said. \u201cWe could see that the whole experience was very reassuring to Jean-Marc, which meant so much to us.&nbsp; We greatly appreciate the excellent services that Dr. Weld and the team at the dental clinic has provided to our son. &nbsp;It\u2019s reassuring to us to have the team here at the hospital and know that they will do what they can to help.\u201d<\/p>\n\n\n\n<p>And for the staff, that\u2019s part of what makes this clinic special.<\/p>\n\n\n\n<p>\u201cI think because we have such a different patient population within this clinic, there is a lot of compassion from the staff,\u201d explained Siobhan.&nbsp; \u201cI&#8217;ve gone in the room myself to hold a patient\u2019s hand because she was so anxious during an extraction. We&nbsp; try and go above and beyond to make sure that they&#8217;re ok.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">\u2018Ok, I\u2019m ready. Let\u2019s get this done!\u2019<\/h3>\n\n\n\n<p>What could have been a difficult or even dangerous appointment ended up being pleasant for everyone, thanks to the teamwork between the dental clinic staff, Jean-Marc and his family.<\/p>\n\n\n\n<p>\u201cGoing to the dentist this time was so important to Jean-Marc,\u201d H\u00e9l\u00e8ne said. \u201cHe forgot all his other behaviours and rituals.&nbsp; Nothing else mattered to him at that point and he was so relieved to be getting his tooth fixed.&nbsp; He just sat there with his sunglasses on and gave the thumbs up as if he were saying, \u2018Ok, I\u2019m ready, let\u2019s get this done!\u2019\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Jean-Marc Lang has autism, epilepsy, a severe developmental delay and ritualistic behaviours that mimic obsessive compulsive disorder (OCD).
