This ensures that individuals who are at high risk for stroke in the Champlain region receive care close to home according to the <a href=\"https:\/\/www.strokebestpractices.ca\/?lang=en\" target=\"_blank\" rel=\"noopener noreferrer\">Canadian Best Practice Recommendations for Stroke<\/a>.<\/p>\n<p>The SPC provides quick assessment and access to diagnostic services and treatment for patients at high risk of suffering a stroke. The clinic aims to reduce the incidence of stroke through:<\/p>\n<ul>\n<li>Early assessment and treatment<\/li>\n<li>Risk factor screening and management<\/li>\n<li>Patient and health-care provider education on stroke prevention and lifestyle changes<\/li>\n<li>Medical care and\/or referral for surgical care<\/li>\n<\/ul>\n<p>The SPC team includes Stroke Neurologists, Stroke Prevention Nurse\/Nurse Specialists, and Administrative Assistants. At the clinic, the patient will meet with the Stroke Neurologist and the Stroke Prevention Nurse\/Nurse Specialist to discuss their risk factors. Patients may receive treatment, have tested and be referred to a specialist. The care is based on individual needs.<\/p>\n<h2>Making an Appointment<\/h2>\n<p>Referral by an emergency room physician, family physician or another medical specialist.
