You\u2019ll talk with a registered nurse or social worker about our referral process. If On Track isn\u2019t appropriate for your needs, we\u2019ll do our best to link you with other resources that are. You can also download a referral form and fax it in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/www.ottawahospital.on.ca\/en\/documents\/2024\/09\/on-track-referral-form.pdf\/\">Download a referral form<\/a> (pdf)<\/li>\n\n\n\n<li>Fax the completed form to 613-737-8318.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">What can I expect when I refer someone?<\/h2>\n\n\n\n<p>We\u2019ll notify you that we\u2019ve received your referral. A registered nurse or a social worker will call as soon as possible to gather further information. If the referral is appropriate, the patient will be given an assessment appointment. Assessments usually take 1\u20132 hours. If possible, family members should come with the patient to meet with a psychiatrist and a primary clinician.<\/p>\n\n\n\n<p>At this point, we\u2019ll decide whether or not the patient will be enrolled in the On Track program. If not, we\u2019ll suggest other treatments, community supports or resources that might be appropriate.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>To help us out:\n<ul class=\"wp-block-list\">\n<li>Bring along investigations\/reports\/discharge summaries, if available.<\/li>\n\n\n\n<li>Bring contact information for anyone you can think of who knows the individual well enough to comment on how his or her behaviour has changed.
