How do I get a copy of my health records or a CD of my Medical Images?<\/a><\/h4><\/div>\n\n\n\n

The simplest way to request and receive a copy of your health records is <\/strong>to make a request in\u00a0<\/strong>MyChart<\/strong><\/a>\u00a0and navigate to\u00a0<\/strong>the\u00a0<\/span>\u201cSharing Hub\u201d through the top left Menu.<\/strong><\/p>\n\n\n\n

Note: While non-urgent requests are completed in the order they are received, TOH records became electronic in November 2009. Therefore, where appropriate, limiting a request to dates after this will minimize processing time.<\/strong><\/p>\n\n\n\n
