Registration Process \u2013 Deadline is May 10, 2016<\/strong><\/p>\n

<\/td>\nCompleted and submitted on-line the registration form by May 10, 2016<\/strong>.\u00a0 Note: there is 30 min allocated to complete the form before the session timed out<\/td>\n<\/tr>\n
<\/td>\nIdentified ALL special requests on the registration form (e.g. late arrival or early departure, PD days, holidays, etc.)<\/td>\n<\/tr>\n
<\/td>\nRegistration e-mail received – sent automatically a few moments after the submission of the registration form on-line.\u00a0 If no confirmation e-mail was received it means that the registration has not been sent.\u00a0 This may be due to the session having timed out if it took more than 30 minutes to complete the registration form<\/em><\/td>\n<\/tr>\n

<\/td>\nE-mailed P.A.R.TY. Coordinator (by the TUESDAY<\/strong> of the week I am attending P.A.R.T.Y) the exact number of students and supervisors attending<\/td>\n<\/tr>\n
<\/td>\nConfirmed your emergency contact number (cell #) – in case weather or extenuating circumstances requires the program to be cancelled that day<\/td>\n<\/tr>\n
<\/td>\nCollected student consent forms<\/td>\n<\/tr>\n
<\/td>\nPrepared to arrive by 9 a.m. and stay until 2:15 p.m. unless otherwise requested on my registration form<\/td>\n<\/tr>\n
