{"id":12744,"date":"2016-12-02T14:06:26","date_gmt":"2016-12-02T19:06:26","guid":{"rendered":"http:\/\/52.229.122.34\/en\/?page_id=12744"},"modified":"2016-12-02T14:06:26","modified_gmt":"2016-12-02T19:06:26","slug":"after-you-leave","status":"publish","type":"page","link":"https:\/\/www.ottawahospital.on.ca\/en\/clinical-services\/deptpgrmcs\/programs\/family-health-team\/booking-an-appointment\/after-you-leave\/","title":{"rendered":"After You Leave"},"content":{"rendered":"<p>Make sure you know what to do when you go home from the hospital or from your medical appointment.\u00a0 If you have any questions please ask your health provider and he\/she will be happy to answer them for you.\u00a0 You should feel confident about all of the statements below before you leave from your appointment:<\/p>\n<ul>\n<li>\u00a0If I have a new prescription, I know about the new medication(s).<\/li>\n<li>\u00a0I know what to do with my current medications.<\/li>\n<li>\u00a0I have requested a refill of my prescriptions if needed.<\/li>\n<li>\u00a0I know if I need further tests and how they will be booked.<\/li>\n<li>\u00a0I know when my next appointment is.<\/li>\n<li>\u00a0I know what to do in case of emergency.<\/li>\n<li>\u00a0I understand the information and instructions I was given.<\/li>\n<li>I will try and take my prescription to the pharmacy right away<\/li>\n<\/ul>\n<h2 class=\"text-center\">\nA Note on Referrals and Consults<\/h2>\n<p>When it is necessary, patients are referred to other medical specialists who are not a part of our team.\u00a0 Your family doctor will determine if you need to see a specialist and will make the arrangements.
