It is preparing for a time when you are unable to make those decisions for yourself. Advance Care Planning is important for everyone; even young, healthy people can suddenly become sick.<\/p>\n<p>Advance Care Planning includes:<\/p>\n<ul>\n<li>thinking about what is important to you<\/li>\n<li>choosing who will make decisions for you if you cannot<\/li>\n<li>sharing your wishes regarding treatments, living situations, etc.<\/li>\n<li>legal decisions, like assigning a Power of Attorney, creating a will or deciding on organ donation<\/li>\n<\/ul>\n<h2>We at the Family Health Team would like to support you through the ACP process.<\/h2>\n<p>We can meet with you and your family to help with<\/p>\n<ul>\n<li>understanding advance care planning<\/li>\n<li>explaining medical information, including diagnoses and treatments<\/li>\n<li>linking you to community resources and supports<\/li>\n<\/ul>\n<h2>Other Tips<\/h2>\n<ul>\n<li>Keep your advance care plan in a safe place<\/li>\n<li>Revisit your ACP whenever your life or health changes. Change it if your wishes have changed.<\/li>\n<li>Share an up to date written copy of your advance care plan with your health care team and the people who are important to you.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>What is Advance Care Planning?
