Our program offers in-patient, out-patient and community services.<\/p>\n<h2>Our Services<\/h2>\n<p>Patients are referred to Geriatrics for assessment of a variety of concerns such as:<\/p>\n<ul>\n<li>Recent changes in physical, mental or functional abilities; falls, impaired mobility, dementia, delirium, change in mood, decreased the ability for self-care<\/li>\n<li>Major changes in support needs, caregiver stress, institutional placement being considered.<\/li>\n<li>Safety concerns \u2013 physical, psychological, social, environmental<\/li>\n<li>Sudden increase in the use of health care services over the last 6 months.<\/li>\n<\/ul>\n<p><strong>In-Patient:<\/strong> Patients who are already in hospital at The Ottawa Hospital may be transferred to our Inpatient Geriatric Medicine Unit at the Civic Campus for assessment and treatment.\u00a0 To be transferred to the unit patients must be referred by an in-patient physician and be seen in consultation by one of our Geriatric Nurse Specialists or Geriatric Physicians.<\/p>\n<p><strong>Out-Patient:\u00a0 <\/strong>Patients are usually referred by their Physician or family.\u00a0 Typically, patients have already been seen in their own home by the Geriatric Outreach Assessor before an appointment is made for the Geriatric Day Hospital.\u00a0 At times, patients will be referred through the Emergency Department (Geriatric Emergency Management Program).<\/p>\n<ul>\n<li><a href=\"\/en\/documents\/2021\/07\/outreach-brochure-nov-07-2.pdf\/\" target=\"_blank\" rel=\"noopener noreferrer\"><em>Geriatric Assessment Outreach Team brochure<\/em><\/a><\/li>\n<li><em><a href=\"\/en\/documents\/2017\/01\/geriatrics_day-hospital-e.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Geriatric Day Hospital brochure<\/a><\/em><\/li>\n<li><a href=\"\/en\/documents\/2017\/01\/gem-brochure-e.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><em>Geriatric Emergency Management brochure<\/em><\/a><\/li>\n<\/ul>\n<p>In partnership with the <strong>Regional Geriatric Program of Eastern Ontario<\/strong>, we offer a wide range of services to help patients and families make decisions to maximize their function and make decisions about their treatment and care.<\/p>\n<p>The Regional Geriatric Program of Eastern Ontario aims to be a leader in the provision of:<\/p>\n<ul>\n<li><strong>Comprehensive, interdisciplinary geriatric assessment and treatment<\/strong> services in partnership with our patients and the community<\/li>\n<li><strong>Education<\/strong> in geriatrics\/gerontology to practitioners, students and community members;<\/li>\n<li><strong>Research<\/strong> into appropriate treatment and services delivery to seniors;
