Over the past 10 years The Ottawa Hospital has embarked on a journey of redesign and reorganization. During this journey, three distinct models have emerged.

First, the Nursing Professional Practice Model was created between 2000 and 2002 due to major system changes in health care and education as well as progress in self regulation. The nursing professional practice environment was redesigned to provide a dynamic infrastructure with policies and processes that support and guide all nurses in the four domains of practice: clinical, education, research and management. Inclusive in this environment is the way the delivery of services, both intra and inter-professional, is organized.

Second, nurses at TOH asked that there be one consistent care delivery model for the patients they care for. The Model of Nursing Clinical Practice (MoNCP) emerged between 2000 and 2002 from a systematic process, wherein a literature review, an environmental scan, surveys and consultations were undertaken and completed. Within this model of care, Registered Nurses and Registered Practical Nurses are accountable for their practice and work to full scope alongside their unregulated care provider colleagues. Implemented in over 110 units at TOH, the MoNCP is the first Canadian Model of Nursing Clinical Practice developed for the challenges of the 21 Century. TOH is currently working with many external partners to implement this model in a variety of practice environments – such as acute, transitional and long term care, as well as rural settings.

Third, the Interprofessional Model of Patient Care (IPMPC ) came about in 2006 at a time when Inter-professional collaboration is high on the policy agenda in Canada . This policy focus will help improve seamless care for patients and families across the continuum of care and will help answer the additional challenges of health human resources shortage.

To date The Ottawa Hospital has developed and is implementing this innovative Inter-Professional practice model of patient care with over 103 teams. TOH has also received inquiries from other health care organizations who wish to implement and or partner with TOH.
