Our goal is to provide each chronic pain patient with the world-class care, exceptional service, and compassion we would want for our loved ones, and to be recognized as a leader in chronic pain rehabilitation in the domains of clinical care, education, quality, and research.

Our Mission

Clinical
The Chronic Pain Management Program is a compassionate interdisciplinary provider of patient-centered care with an emphasis on providing assessment, education, and promotion of self-management of chronic pain and associated disability for patients in the Champlain LHIN.

Academic
The Chronic Pain Management Program is committed to education of future and present health-care professionals within an interdisciplinary model of care.

Quality and Research
The Chronic Pain Management Program is committed to evidence-based patient care through development, sharing, and application of new knowledge, clinical approaches, and digital solutions.

Our Values

Compassion
One of the primary aims of the Chronic Pain Management Program is to empower patients and foster increased hope by providing them with strategies and principles to enhance functional abilities. Through this program, the patients are encouraged to make positive change and healthy choices to improve their quality of life.

A Commitment to Quality
The Chronic Pain Management Program is founded in an evidence-based, patient-centered care model with a focus on treating the whole person. The program has a firm commitment to quality assurance and monitoring the success of the patients during and after they attend the group based interventions program.

Working Together
An interdisciplinary approach is taken in the chronic Pain Management Program with the aim of treating the whole person. The group structure of its educational model allows for the patients to learn from both the expertise of the clinicians and the shared experiences of fellow participants.

Each patient in the Chronic Pain Management Program is treated with courtesy, dignity and respect. The interdisciplinary team is sensitive to the individual needs of patients and their families, including stages of development, lifestyles, and culture.
