The Ottawa Hospital is pleased to accept nominations for the fourth Annual Board of Governors Volunteer Recognition Awards. This award celebrates those who have demonstrated a commitment to volunteerism and whose gift of time is invaluable to our patients and staff.

Where can I find the Call for Nominations Form?
How does it work?
Complete the Call for Nominations Form and describe why you believe this
individual is deserving of The Ottawa Hospital Board of Governors Volunteer Recognition Awards
and the impact they have had on the unit/area served.
Any supporting materials, such as achievements, newspaper articles or letters written by or
about the nominee, would be helpful.
Five individuals will receive The Ottawa Hospital Board of Governors Volunteer Recognition Award
at the Annual General Meeting on June 24, 2009. All nominees and the five Award recipients will
be notified in early June.
What is the deadline for the submission of nominations?
All nominations must be received by April 30, 2009.
Who can nominate a volunteer?
The nomination process is open to all employees, whether they are front-line health care workers or
support staff. Volunteers and patients are also welcome to submit a nomination.
Can a volunteer recognize another volunteer through this nomination process?
Yes.
